Congress included a provision in the Agricultural Act of 2014 that allowed colleges and state agencies to grow and conduct research on hemp in states where it is legal. Hemp production in Kentucky, formerly the United States' leading producer, resumed in 2014. Hemp production in North Carolina resumed in 2017, and in Washington State the same year. By the end of 2017, at least 34 U.S. states had industrial hemp programs. In 2018, New York began taking strides in industrial hemp production, along with hemp research pilot programs at Cornell University, Binghamton University and SUNY Morrisville.

What is often classified as “medical marijuana” is likely to contain a psychoactive compound THC (what gets people “high”), and these products are dangerous to dogs. Separation of hurd and bast fiber is known as decortication. Traditionally, hemp stalks would be water-retted first before the fibers were beaten off the inner hurd by hand, a process known as scutching. As mechanical technology evolved, separating the fiber from the core was accomplished by crushing rollers and brush rollers, or by hammer-milling, wherein a mechanical hammer mechanism beats the hemp against a screen until hurd, smaller bast fibers, and dust fall through the screen. After the Marijuana Tax Act was implemented in 1938, the technology for separating the fibers from the core remained "frozen in time". Recently, new high-speed kinematic decortication has come about, capable of separating hemp into three streams; bast fiber, hurd, and green microfiber.

The Cannabis plant has a history of medicinal use dating back thousands of years across many cultures. The Yanghai Tombs, a vast ancient cemetery (54 000 m2) situated in the Turfan district of the Xinjiang Uyghur Autonomous Region in northwest China, have revealed the 2700-year-old grave of a shaman. He is thought to have belonged to the Jushi culture recorded in the area centuries later in the Hanshu, Chap 96B. Near the head and foot of the shaman was a large leather basket and wooden bowl filled with 789g of cannabis, superbly preserved by climatic and burial conditions. An international team demonstrated that this material contained tetrahydrocannabinol, the psychoactive component of cannabis. The cannabis was presumably employed by this culture as a medicinal or psychoactive agent, or an aid to divination. This is the oldest documentation of cannabis as a pharmacologically active agent.

In fact, the U.S. Food and Drug Administration (FDA) approved Epidiolex (a drug made with a purified form of CBD oil) in June 2018 for the treatment of seizures associated with two rare and severe forms of epilepsy in patients 2 years of age and older. These two epilepsy forms are known as Lennox-Gastaut syndrome and Dravet syndrome. Epidiolex is the first FDA-approved drug that contains a purified drug substance derived from marijuana.
A study by Henquet and colleagues (2004) substantially replicated both the Swedish and Dutch studies in a 4-year follow-up of a cohort of 2437 adolescents and young adults between 1995 and 1999 in Munich. They found a dose–response relationship between self-reported cannabis use at baseline and the likelihood of reporting psychotic symptoms. As in the Dutch cohort, young people who reported psychotic symptoms at baseline were much more likely to experience psychotic symptoms at follow-up if they used cannabis than were cannabis-using peers without such a history.
The health consequences of cannabis use in developing countries are largely unknown beacuse of limited and non-systematic research, but there is no reason a priori to expect that biological effects on individuals in these populations would be substantially different to what has been observed in developed countries. However, other consequences might be different given the cultural and social differences between countries.
Heavy, long-term exposure to marijuana may have biologically based physical, mental, behavioral and social health consequences and may be "associated with diseases of the liver (particularly with co-existing hepatitis C), lungs, heart, and vasculature". However, maternal use of marijuana during pregnancy does not appear to be associated with low birth weight or early delivery after controlling for tobacco use and other confounding factors. A 2014 review found that while cannabis use may be less harmful than alcohol use, the recommendation to substitute it for problematic drinking was premature without further study. Various surveys conducted between 2015 and 2019 found that many users of cannabis substitute it for prescription drugs (including opioids), alcohol, and tobacco; most of those who used it in place of alcohol or tobacco either reduced or stopped their intake of the latter substances.
